If I'm not mistaken Sgt. Splatters has (or had) a simple ladder into the second story on one side of the field. People didn't use it often as it was slower than the stairs but it helped in stallmates up there.

With regards to the ladder note: at UA they have (or at least had) a slanted ladder/staircase going up to the second level of their "tower" thing. Worked really well and had railings on both sides. Could be looked at as a good option, especially if there is a wall to the back of the person climbing said ladder-case.
Definitely have internal as well as external methods of getting to second story levels on kill houses though. Adds another dynamic to the defence of the position. Will have more suggestions to follow.
